Mobile Treatment Team

The mobile treatment team, composed of a medical prescriber, therapist, substance use specialist, peer support specialist, nurse, and administrative specialist, offers intensive outpatient services to adults in the Mid-Shore communities who face challenges engaging in traditional outpatient mental health services. Clients often have limited access to community resources and support networks.

How it Works

  • Team members meet several times throughout the week to collaborate and ensure that their clients are receiving the care that they need.

  • Clients receive regular psychiatric medication management services and are provided with education, medication planning, symptom management, and help addressing somatic health issues.

  • Team members assist clients in becoming linked to community resources, maintaining entitlements, developing healthy money management skills, locating and maintaining housing, and finding employment.

  • A weekly WRAP group is facilitated with clients to promote positive peer supports, healthy communication patterns, and the ability to utilize the skills they are equipped with in meetings.

  • Clients receive evidenced based individualized therapeutic, and substance use counseling services to promote wellness by developing coping skills, treatment planning, navigating grief and relationships, stress management techniques, behavior modification methods, and addiction education and counseling.

Eligibility

Individual has a priority population mental health diagnosis with history of failed attempts to successfully engage in traditional outpatient mental health treatment and:

  • Over the age 18.

  • Frequent utilization of mobile crisis/psychiatric treatment for mental health reasons

  • Arrests/legal matters related to psychiatric illness.

  • Experiencing chronic homelessness
